JAMS Athletics

JAMS Middle school is transitioning to FinalForms, an online system, that boosts compliance and safety. FinalForms allows you to complete and sign forms from your computer, table, or mobile device.

FinalForms saves data from season-to-season and year-to-year, meaning you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving parents/guardians time if you have multiple children.

You may log in any time to update information including emergency medical contacts and information. You will be required to sign your forms once per year and anytime you make an update.

A parent must create their FinalForms account first before your student is able to create their account. We ask ALL parents/guardians of athletes to use FinalForms.

Boys Soccer
Soccer tryouts are 3 days: 3/18, 3/19, and 3/21, 4-5:30 on the JAMS field. Students should attend all 3 days of tryouts. Varsity practice will be held on Tuesday and Thursdays, JV practice dates are not confirmed. Families who are signed up on Final Forms will receive information from Coach Ochoa soon.

Girls Volleyball
Volleyball tryouts are 3/19 and 3/21, 4-5:30 in the JAMS gym. Students should plan on attending both tryouts. Practice will be held on Tuesdays and Thursdays. Families who are signed up on Final Forms will receive information from Coach Johnson soon.

Track Co-ed
Track will meet on Tuesdays and Fridays from 4:00 – 5:30 (students meet near the JAMS recess field and head to Hale’s track as a team). There is no tryout process, all students who are cleared on Final Forms can participate. Students registered on Final Forms will get more information from Coach Park.

Spring Ultimate Frisbee is not a school-sponsored sport. Big Cat Ultimate registration links will be available prior to the spring season.

Final Forms Instructions:

Parents/Guardians will need to create an account first through the Parent Portal.

Once you are in, you will need to add your student.

You will need to register your student with a non-SPS email for your student. This can be another adult email, your student’s non-SPS email, or create an email specifically for FinalForms.

Accessing the Appropriate Forms

You will add forms by clicking on the Add Sport/Activity Link.

This will pull up a page where you can find the Sport/Activity you need. Clubs are under Extracurricular Activities. Click on Update sports/activities.

Completing the Forms

For all forms, Athletic & Extracurricular Activities that need to be completed by a parent/guardian, must be completed by the adult first. Then the student will get an email prompting them to complete their portion of the form.

The student will then follow the link in the email to create their own account in FinalForms that they access through the Student Portal.

Physicals

All students need a doctor-signed physical uploaded onto Final Forms before they can participate in sports.

Physicals are valid for two years from the exam date. Check with Tahera Kapasi tekapasi@seattleschools.org to see if your student has a current physical on file at JAMS. Nathan Hale has a Teen Health Clinic that can provide sports physicals for our students. For more information, call 206-302-1840.


SPS Athletics

Athletic Registration

We are using FinalForms for online athletic registration. If you previously had an account, you can log in and all your info will still be there!

If you have not created an account, please follow the instructions below. A parent/guardian must create their FinalForms account first, before the student is able to create their account.

  1. Parents/guardians, go to the FinalForms website
  2. Type your name, date of birth, and email and then click register
  3. Check your email for a FinalForms email, and click confirm your account in the email text.
    • You will receive an email within 2 minutes prompting you to confirm and complete your registration. Remember to check your spam folder. If you still do not see the FinalForms email, please email support@finalforms.com.
  4. Create your new FinalForms password and click confirm account.
  5. Click register student for your first child. Add your student and their email.

Physicals are valid for 24 consecutive months

  • The History document (Page 1) should be completed and remain with your provider
  • The Exam document (Page 2) should be completed and remain with your provider
  • The Medical Eligibility document (Page 3) should be the only item submitted to your school

Student-athletes must have health insurance to play a sport. If not covered by a parent/guardian’s insurance plan, insurance can be purchased through SPS.

Digital Tickets for High School Games

SPS home games (matches played at SPS high schools) can be purchased online via SPS HomeTown Ticketing Box Office.

This includes all regular season games for football and basketball. It also includes post-season games for volleyball, basketball, wrestling, gymnastics, baseball, soccer, softball (slowpitch and fastpitch), and track/field. Learn more about SPS Digital Athletic tickets.

Please note: tickets are posted under the HOST school (For example, West Seattle High School hosting Garfield High School would appear under West Seattle.)
